We value the safety of its players and volunteers. We want to ensure that our organization upholds the highest standards in providing a safe secure environment for our children to play field hockey. All individuals working, volunteering or who have direct contact with children aged 19 or under must submit a criminal records check (CRC) to the club. I, recognizing that field hockey can be a hazardous sport, hereby forever release, discharge and hold harmless CJFH & CFHA representatives and agents for any injury, loss damage to person or property howsoever caused, arising out of or in connection with my child taking part in the games and activities which he/she is hereby entering. I acknowledge that I have read and understand the Membership Information attached .
